With the transit visa and a driver, you can do the Pyramids and the Grand Egyptian Museum and cross into Islamic Cairo — the Citadel, Khan el-Khalili bazaar — with time for a proper dinner, and still make your flight. It's the first layover length where you can pair the ancient sights with the living city rather than choosing.
Sixteen hours is where a Cairo layover becomes a genuine two-act day: the pharaonic west side in the morning, the medieval city in the afternoon. Sequenced right — and it has to be sequenced right, because of the traffic — it's one of the best long layovers anywhere.

The two-act day: ancient, then medieval
The morning is Giza and the Grand Egyptian Museum — the pyramids, the Sphinx, and Tutankhamun's treasures in the vast new museum beside the plateau. The afternoon crosses to a completely different Cairo: the Citadel of Saladin on its hill with the alabaster Mohamed Ali Mosque and a city-wide view, then down into Khan el-Khalili, the centuries-old bazaar of lantern-lit alleys, coppersmiths and coffee houses. A dinner in the old city — grilled meats, mezze, a mint tea in a historic café such as the centuries-old El Fishawy in the heart of the bazaar — is the natural close before the drive back. It's a lot to fit in, but 16 hours genuinely holds it if you keep Giza in the morning and don't let any single stop overrun.
The slower alternative, if you'd rather not rush
Sixteen hours holds the two-act day comfortably, but it doesn't have to be packed. A perfectly good alternative is to slow the ancient half right down — a long, unhurried morning and lunch at Giza and the Grand Egyptian Museum, going inside a pyramid, lingering at the Sphinx and the panorama — and then pick just one afternoon stop in the old city (the Citadel for the view and the mosque, or Khan el-Khalili for the bazaar, not both) before an early dinner and a relaxed drive back. If it's your first time in Egypt and you'd rather feel one place deeply than tick four, this is the better use of 16 hours than a forced march. There's no prize for cramming; the pyramids reward the time you give them.

Keep the return sacred
The risk on a big day is letting the afternoon run long. Fix your "leave the city" time against worst-case evening traffic and hold to it — Cairo's roads are heaviest 3–7pm, and the airport run from the old city is not quick. A driver for the whole day, tracking your flight, is close to essential at this length.
